WA Primary Health Alliance (WAPHA) is continuing to undertake a number of activities to strengthen the way we perform our commissioning function across the entire commissioning life cycle. Whilst this includes a greater focus on measurement and evaluation, we are committed to internal change focused on system and process transformation. This change also incorporates a more clearly defined and structured program approach here at WAPHA with intent to strengthen our procurement/commissioning aligned to under-served communities and locations, and better supporting our engagement with providers.

 

Much of this change cannot be achieved alone and will also mean change for our commissioned service providers. You will have already seen some of this change being introduced recently through our approach to contract management at the program and activity level – we understand that this has been challenging for some, and we will continue to review and refine the process with you to achieve the right balance. Another change that you will also see through our commissioning is a focus on activities of multidisciplinary team care in support of general practice – this will require a slight paradigm shift for some providers in the context of ‘how’ a service can be provided that will differ from the norm.

 

Thank you for the work that you and your teams do each day in support of our communities to strengthen access and equity to primary health care services in Western Australia.

WA Primary Health Alliance launches Care of Chronic Conditions Strategy

WAPHA is committed to enhancing the quality and accessibility of person-centered primary health care for people with chronic conditions who need it the most.  Our newly released Chronic Conditions Care Strategy (2025-2027) outlines the principles underpinning our Chronic Conditions Program framework and associated care models.  Developed in line with our Strategic Plan (2024-2026), the Strategy provides an overview of WAPHA’s priorities and strategic directions for chronic conditions care.

Head to Health evolves to Medicare Mental Health 

Head to Health has evolved to Medicare Mental Health, making face-to-face, digital and telephone initiatives more consistent with other trusted health services funded by the Australian Government.

Medicare Mental Health helps people access the mental health and wellbeing services that are right for them. It also makes it easier for GPs and other health professionals to support the mental health of the people they look after, and those who support them, through improving access to, and coordination of, mental health services in their local community.

Medicare Mental Health includes:

 

  • A digital platform - The website includes evidence based digital mental health resources, apps and online programs. This includes dedicated online forums, peer support services, chat and email services.
  • National phone service (1800 595 212) - Offers free confidential assessment and referral for anyone seeking help for their mental and emotional wellbeing and/or wanting to support a patient, or someone they care about.
  • A network of Medicare Mental Health Centres - These services are free and confidential for people aged 18 and over. No referral, appointment or Medicare card is needed.

Changes to eligibility for aged care services for younger people 

From 1 July, the new Aged Care Act will limit entry to Australian Government-funded aged care services to:

  • People aged 65 years and over.
  • Aboriginal and Torres Strait Islander people with care needs aged 50 years or over.
  • People with care needs who are homeless or at risk of homelessness aged 50 years or over. 

Ability First Australia’s (AFA) Younger People in Residential Aged Care (YPIRAC) System Coordinator Program  is a nationally funded program. The program focuses on younger people who are not eligible for the National Disability Insurance Scheme (NDIS). It’s designed to reduce the number of younger people at risk of entering, and living in, residential aged care.  

 

Changes from 1 July AFA YPIRAC System Coordinator teams will continue to work with younger people to: 

  • Test NDIS eligibility. 
  • Work with state and territory agencies to identify appropriate services to meet the needs of younger people.  

AFA will not refer younger people to My Aged Care for an aged care needs assessment.  

 

Resources supporting palliative care at home for Aboriginal people 

When care at home is preferred, it can be provided to help connect family with culture, community, Country and the spiritual wellbeing of Aboriginal people. caring@home has released new and updated resources to support your Aboriginal patient and their families receiving at-home palliative care.

 

The resources feature plain language, clear images and tailored Aboriginal and Torres Strait Islander artwork. Clinical services can print hard copies of these free resources to provide to families when needed.

 

caring@home is a National Palliative Care Project, funded by the Australian Government. It aims to increase access to quality and timely end-of-life care for home-based patients through supporting health professionals with evidence-based resources and education.

Perinatal mental health checklist in 40 languages

Perinatal Anxiety & Depression Australia has released an online mental health checklist translated into 40 languages. Designed specifically for new and expecting families, it was translated in collaboration with each language group to ensure it is culturally sensitive. 

The checklist allows mums, dads, non-birth parents and family to self-assess how they are feeling and offers pathways to support where needed. Printed resources in 40 languages can be ordered, free, to have available in health care services.

Peel Youth Mental Health Navigation and Coordination

The Initial Assessment and Referral Decision Support Tool helps general practitioners and clinicians to recommend the most appropriate level of care for a person seeking mental health support.

Being trauma responsive

This workshop discusses talking safely with someone who is impacted by trauma and teaching simple strategies to support recovery, respond to someone experiencing triggers and improve the experiences of people impacted by trauma when seeking support.

Dealing with Depression in Rural Australia is developed specifically to address the needs of clinicians delivering health care for people presenting with depression in a rural setting.
