New Quality Improvement (QI) Toolkit for General Practice in Aged Care Incentive now available

The General Practice in Aged Care Incentive (GPACI) Quality Improvement (QI) Toolkit provides practical resources and guidance for general practices and Aboriginal Community Controlled Health Services, to support planned, preventative care and enhance coordination with multidisciplinary teams to improve patient outcomes and care continuity.


Practices can utilise the toolkit to streamline their workflows, access a range of helpful resources, and implement continuous QI activities, ultimately fostering a patient-centred approach in aged care settings. This toolkit has been developed by PHNs working together nationally through the PHN Cooperative, the National Improvement Network Collaborative and the National MyMedicare PHN Implementation Program.


Residential aged care providers may wish to promote the toolkit with their primary care providers.

After-hours support resources now available for aged care

WA Primary Health Alliance (WAPHA) is helping residential aged care homes (RACHs) with enhancing out-of-hours action planning.


We have developed a suite of resources to assist aged care homes in self-auditing and improving the quality and sustainability of after-hours care to their residents.


These resources was made possible through consultation with aged care workers, who provided advice on the content and format. A key stakeholder who assisted in the development and trial of the resources was Hall & Prior executive director Dr Kristi Holloway.

“Being able to deliver high quality care in a resident’s own environment after hours where feasible is the main goal. These resources have demonstrated that they can help clinicians in aged care to be able to achieve that.” - Dr Kristi Holloway.

The resources are designed to guide users to bodies of knowledge. The inclusion of an audit tool helps facility managers identify areas that they might consider enhancing, such as escalation of care, available services, and digital capability.

Specialist panel discusses enhancing access to care using telehealth carts

WAPHA recently hosted a telehealth networking conference and panel discussion with Dr. Belinda Wozencroft, Practice Principal at View Street Medical, Gordon Stacey, Director at View Street Medical, Rebecca Glass, Telehealth Project Clinical Lead at MYVISTA, and Shallyse Loveland, Nurse Practitioner at Hollywood GP.

The panel provided valuable insights into the benefits of using telehealth carts available in aged care homes across WA. They highlighted how telehealth carts can improve residents’ access to care, minimise travel needs for GPs, and enable seamless connections with wider care teams.

Play your part in protecting yourself and those your care for through timely staff vaccinations 

Protecting yourself as a provider of aged care through vaccination and additional doses helps to protect the people you care for, your family and community from the impacts of COVID-19 including serious illness, hospitalisation and death. 


Unlike some other respiratory viruses, which tend to peak in winter, COVID-19 has been shown to spread all year-round, regardless of the season. New variants may emerge at any time, potentially increasing the risk during our warmer months. 


Although the Australian Government is not mandating COVID-19 vaccinations for aged care workers, people working in aged care, 18 years of age or older are encouraged to get a COVID-19 vaccination every 12 months. In the upcoming flu season, aged care workers can receive their COVID-19 and influenza vaccines together.

Government updates

WA Virtual Emergency Department - Bunbury expansion

Since its launch in September 2023, the WA Virtual Emergency Department (WAVED) has been progressively expanded to give more patients the option of having a virtual consultation with a specialist emergency physician in the comfort of their own home.


Residents of aged care homes were prioritised for early access to WAVED, with virtual consultations being offered prior to ambulance dispatch for low acuity triple zero (000) calls. WAVED now also accepts referrals for community-dwelling adults from St John Ambulance WA and a small number of additional agreed referrers in the Perth metropolitan area.


In partnership with St John Ambulance WA, WAVED will expand into the Bunbury Hospital catchment from late February 2025. Eligible patients referred by St John Ambulance WA (either before or after ambulance dispatch) will be assessed via a telehealth platform, providing earlier access to emergency specialist assessment and where appropriate, electronic prescription and/or referral to services that support care in the community.


It is important GPs are aware of this service, as WAVED clinicians may occasionally get in touch to discuss a patient’s care and involve the GP in decision making. All relevant clinical information is documented in a discharge summary which is securely transmitted to the nominated GP and uploaded to the patient’s My Health Record, with a copy provided directly to the aged care home.


Expansion of WAVED will continue throughout 2025, with an initial focus on developing a referral pathway from the healthdirect advice line and establishing a paediatric sub-service.

Refer your patients to the new Single Assessment System for aged care

The Australian Government Department of Health and Aged Care is establishing a Single Assessment System to make it easier for older people to enter aged care and access different services as their needs change.

On 9 December 2024, the Single Assessment System workforce brought together and replaced the Regional Assessment Service, Aged Care Assessment Teams and independent Australian National Aged Care Classification assessment organisations.

My Aged Care support plans available in My Health Record

My Aged Care support plans developed or updated from 9 December 2024 can now be made available in My Health Record. This integration of My Aged Care to My Health Record is the foundational connection between aged care and health systems.

During aged care assessments, assessors will ask older people if they authorise sharing their support plan with My Health Record. Older people can provide or withdraw authorisation at any time. The support plan shows which aged care services they qualify for based on their needs and abilities.

Residential aged care providers to prepare for the allocation of Places to People

The Australian Government Department of Health and Aged Care is making changes to the aged care system in response to the recommendations of the Royal Commission into Aged Care Quality and Safety. The new Aged Care Act will introduce a new system that allocates a residential place directly to an older person, instead of an aged care provider.


For older Australians, this reform will benefit them with more choice and control over which provider delivers their care. Providers will no longer be restricted by the number of places they have been allocated. This will enhance service quality, innovation and financial viability for providers. In preparation for this new approach, the Government has discontinued the Aged Care Approvals Rounds.


Until the change commences, providers who are ready to deliver care immediately can apply to the Department of Health and Aged Care for places. To prepare for the upcoming change, providers will need to write off any bed licenses on their balance sheets before the new arrangements begin.

End of Life Law for Clinicians (ELLC) has launched a free online aged care course that focuses on end of life law in aged care, and health professionals’ legal roles and obligations. A new module has also been added for health professionals in all health settings - Module 14, End of Life Law in Aged Care.

Education: Aged care learning platform is available


An aged care learning information solution (ALIS) has now been included in the WAPHA after hours toolkit.


ALIS is provided by the Aged Care Quality and Safety Commission and offers education and training to help aged care health care workers understand their obligations under the Aged Care Quality Standards. The knowledge gained can potentially support processes to enhance after-hours action planning.

The Government Provider Management Systems (GPMS) guide provides a brief introduction to the digital systems enabling provider operations under the new Aged Care Act (new Act effective 1 July 2025).


It includes information about how all digital changes come together to support providers across the provider journey including providing data fields, categories and rules that are critical in enabling software system development.

Toolkit: New campaign promotes Star Ratings for aged care homes

Star Ratings help older people to research and compare the quality of care, safety and services of aged care homes. This helps them to make more informed choices about the care that best meets their needs.

A toolkit has been developed to support health professionals with talking to patients about residential aged care planning and how Star Ratings can help. The kit includes conversation talking points, a printable poster and brochure, fact sheets, translated resources and more.

Resource: Seniors Information Resource now available

The Seniors Information Resource is an action of the State Seniors Strategy 2023-2033 and was developed in response to feedback from older people across WA who identified the need for clear, accessible information about services, programs and concessions available.

It includes information about housing, health, aged care services, elder abuse, technology and communication, transport, connecting with community, end of life planning and a range of other topics.
